Died, according to the Official Records, at 86 Court Road, Orpington though other sources say she was killed at her home, 88 Kynaston Road, Orpington. Ivy was the 60595th and last civilian killed in Britain by enemy action in the Second World War. At 16.54 hrs on 27th March 1945, the 1115th and final V2 to land in the country exploded nearby, killing her. Apparently she had just gone into the kitchen to boil a kettle. Her husband, who had remained in the front room of the house, survived.

Ivy was laid to rest in All Saints Church but her grave remained unmarked until Remembrance Sunday in 1989, when a memorial service was held. Every Remembrance Sunday, after the service in All Saints, the Minister and congregation process to the Canadian corner to remember the Canadian soldiers who are buried there and afterwards to Ivy's grave to pay homage to her and all civilians. There is also a sequoia tree planted in the Priory Gardens, and a plaque, near the Library, in her memory.
