Florian D. Michalak, 58, of 3 Carolyn Lane, Glenville, died yesterday at Ellis Hospital after a short illness.

Born in Dunkirk, Mr. Michalak served many years as an insurance agent for Prudential Insurance Co., going into business last year as an independent insurance broker. He was a communicant of St. Joseph's Church, Scotia, and a member of its Ushers Society and the First Friday Luncheon Group of Schenectady County. He was a captain in the Army in World War II.

Survivors include his wife, Mrs. Mary Jane Frey Michalak of South Carolina; Robert Michalak and Edward Michalak, both of Dunkirk; two sisters, Mrs. Helen Brown of Dunkirk and Mrs. Amelia Schafer of California; and several nieces and nephews.

Services will be conducted at 8:30 a.m. Friday in Lasak Funeral Home, Scotia, followed by a Mass of Christian burial at 8:45 in St. Joseph's Church. Graveside military services will be conducted Friday afternoon at Cypress Hills National Cemetery, Brooklyn.

The funeral home will be open after 7 p.m. tomorrow. Contributions may be made to the St. Joseph's Church Building Fund.
