Notes: During the Korean War, Thomas F. Kozak, an Air Force pilot, flew 100 combat missions. His many decorations included the Distinguished Flying Cross. He had been under orders to go to Vietnam, and had been sharpening up his gunnery capability at Nellis Air Force Base in Nevada. He was killed in a crash. The 37-year-old fighter pilot is survived by a wife, two children, his father, Sam, of 3113 Ruby Ave., and three sisters. Maj. Kozak was a native of Cleveland. Son of Maria and Samuel (Szymon) Casimir Kozak.
