On June 13, 1966, Fox Company, 2/9 Marines were conducting a search and destroy mission in Quang Nam Province, RVN, during Operation Liberty, a search and clear operation beyond the secured areas of Monkey Mountain and environs west of Da Nang. At approximately 5:35 PM, Fox Company was set up in a command post five miles northeast of the Loc-Ai Nghia Bridge when it received an unknown-type explosive device. The blast killed PFC Jonathan C. Roker and wounded two others, including LCPL Paul M. Haslinger, who died the following day. It was later speculated that the explosion was caused by a Vietcong rifle grenade.
