Sally K. Martin Achterberg, 46, of East Peoria passed away Monday, Nov. 3, 2014, at the Apostolic Christian Home of Eureka.

She was born on Dec. 6, 1967, in Peoria, Ill., a daughter to James and Helen Woertz Martin. She married Scott Achterberg on Aug. 30, 1991, in Peoria, Ill.

Surviving are two sons, Jake and Heathe Achterberg of East Peoria; one daughter, Ledy Carm of East Peoria; her parents, James and Helen Martin of Eureka; and five brothers, Jim Martin of Longmont, Colo., Tim (Linda) Martin of Eureka, Tom Martin of Highland Park, Dennis (Lori) Martin of Redmond, Ore., and David (Bonnie) Martin of Newman, Ga.

She was preceded in death by her grandparents.

Sally was an employee at OSF Healthcare and St. Jude Children's Research Hospital .

She and her dad were successful at showing Tennessee Walking Horses in several states.

Burial will be in the Goodfield-Congerville Apostolic Christian Cemetery.
