Johanna (Demel) Ondrej
I originally had her as Johanna Farda, but I'm unsure where I got that maiden name from. According to her death certificate, Johanna Ondrej died on November 27, 1928 at the San Antonio State Hospital in San Antonio, Bexar County, Texas. Her cause of death is listed as Pulmonary Tuberculosis. Her parents are unknown, born in Moravia. The information is from the records of S. A. State Hospital. We were unable to get a maiden name from this document. In search of her maiden name, I looked at the death certificates of her children. Only two had her maiden name. Those two were Alois and August. They both had her maiden name as Janie Demel.
In search of documentation for Johanna's maiden name:
1) Death certificate of son, Alois Ondrej, his mother was Janie Demel.
2) No death certificate found for Julius Ondrej.
3) Death certificate of son, August Ondrej, his mother was Janie Demel.
4) Death certificate for son, Frank Ondrej, has father as Joe Ondrey, doesn't know the mother's name. Informant was August Ondrej.
5) Death certificate for daughter, Ludmilla Agnes (Ondrej) Woytek, doesn't have father or mother's names. Informant was Maurice Woytek.
6) Death certificate of daughter, Albina (Ondrej) Hornak, gives mother as Johanna Unknown. Informant was Adolf Hornak.
Her headstone has Janie Ondrej. I'm unsure at this time where the maiden name of Farda came from, but I feel reasonably sure it is inaccurate and her maiden name is Demel, based on the death certificates of two of her children, Alois and August. I was unable to find an obituary for her.
